TERMS AND CONDITIONS AND ANSWERS TO FREQUENTLY ASKED QUESTIONS:

1. NO APPOINTMENTS ARE MADE FOR THIS EVENT. It is first come, first served. Since this is a first come, first served event, there is no guarantee everyone will receive services.
2. IF YOU HAVE DENTAL INSURANCE, YOU ARE NOT ELIGIBLE.
3. You must be 18 years or older to receive treatment.
4. You must have a current, government-issued photo ID.
5. You must provide a list of all current medications.
6. One service will be provided per patient-your choice of cleaning, filling, or extraction. Once your service is completed, and if time permits, you may opt to get back in line to receive an additional service.
7. Because of the high number of patients we will see, we regret that we cannot offer extensive surgical extractions or nitrous oxide (laughing gas) at this event.
8. We will begin registration at 7:30 am SHARP. Once you arrive, you will sign in to establish your place in line. NO APPOINTMENTS ARE MADE FOR THIS EVENT.
9. Once you have signed in and your registration has been completed, you may return to your car to wait or wait outside under the tents we will have set up.
10. NOTE: If you have excessively high blood pressure, certain health conditions, or take certain medications, you may be disqualified from having treatment. This will be reviewed with you during the registration process.
11. We regret that we do not have enough space in our lobby for everyone to wait inside. We have room for only 10 people in the lobby at any time. Others will need to remain outside to wait. Only the patient is allowed inside; companions/drivers/children must wait in the car.

12. There is absolutely NO SMOKING allowed anywhere on the premises, including all areas of the parking lot. This includes cigarettes and e-cigarettes. This is a healthcare facility, so this rule will be strictly enforced. Anyone caught using these products on our property will be disqualified.

13. This is a first come, first served event. We will begin seeing patients at 7:30am and will take the last patient at 4pm. There is no guarantee everyone will receive services.

14. Once you have signed in, completed your registration, and gotten your number in line, you may return to your car to wait. When it is your turn, we will call your cell phone. Make sure your phone is ON and CHARGED. You will have 10 minutes from the time we call to get into our lobby. If you don’t answer and don’t appear in 10 minutes, you will forfeit your turn. If no phone, you need to wait outside.

15. No prescriptions for any medications, including antibiotics and pain medications, will be written for anyone during this event. Ibuprofen will be provided if needed.
